Stroke Recovery and Rehabilitation

Stroke recovery at our skilled nursing facility focuses on intensive, interdisciplinary rehabilitation designed to restore function and improve independence. Patients admitted to our center after experiencing a stroke are provided with an individualized therapy program tailored to their medical condition, tolerance, and personal goals.

Residents recovering from a stroke typically participate in Physical Therapy, Occupational Therapy, and Speech Therapy, working collaboratively to address any physical, cognitive, communication, and swallowing challenges they may be experiencing.

Key Program Elements:

Therapy services may include:

  • Spasticity and hypertonicity management

  • Pain management

  • Orthotics and assistive device assessment and training

  • Management of musculoskeletal complications related to stroke

  • Mobility and gait training

  • Swallowing evaluation and dysphagia management

  • Cognitive rehabilitation

  • Activities of daily living training, which include bathing, dressing, and self-care

Therapy evaluations are typically completed within the first few days of admission, allowing the interdisciplinary team to develop a patient-specific treatment plan. Ongoing patient and caregiver education is incorporated into treatment sessions to support recovery and long-term success.

Interdisciplinary Care Team

Care is provided by a coordinated team that may include:

  • Physician (including in-house Physiatrist)

  • Nurse Practitioner or Physician Assistant

  • Registered Nurses and Licensed Practical Nurses

  • Physical Therapists

  • Occupational Therapists

  • Speech-Language Pathologists

  • Respiratory Therapists

  • Licensed Dietitians

Medical Management

Clinical oversight emphasizes prevention and management of secondary complications, including spasticity, pain, depression, and blood pressure control, ensuring medical stability throughout the rehabilitation process.

Discharge Planning & Transition Home

From admission, the interdisciplinary team works closely with residents and families to prepare for discharge. Social Services coordinates communication with the resident’s primary care provider and arranges post-discharge services such as home nursing, physical therapy, occupational therapy, and speech therapy as needed.

Therapy Intensity

Rehabilitation focuses on improving functional ability through individualized, goal-driven therapy such as gait and mobility training, activities of daily living retraining, cognitive therapy, and communication and swallowing interventions—tailored to each resident’s capacity and endurance.
