Skin and Wound Care Program
The Landmark Management Solutions Skin and Wound Program focuses on the prevention and management of a wide range of conditions, including complex pressure injuries, surgical wounds, arterial wounds, venous wounds, and diabetic wounds.
Our program follows a multidisciplinary approach to the prevention and treatment of skin impairments. This team includes wound-certified physicians, certified or specialty-trained wound nurses, therapists, dietitians, and social services professionals. Staff nurses participate in wound care essentials training, while Certified Nursing Assistants complete skin care essentials training to support early identification and prevention.
Landmark Management Solutions has partnered with Smith & Nephew to provide cutting-edge, evidence-based wound care products designed for the treatment of acute, chronic, and surgical wounds. These advanced products support dermal repair and tissue regeneration while helping prevent complications such as pressure injuries and surgical site infections.
Wound-certified physicians and nurse managers round weekly, or more frequently as needed, to assess wounds and determine appropriate treatment goals. Nurses also provide wound care education to short-stay patients to support continuity of care and proper wound management after discharge.
